Report a Health or Safety Concern
Employees can report a safety concern or safety suggestion online directly to the Health and Safety Coordinator.

A Concern is defined as a potential or existing hazard which presents risk to the Health or Safety of individuals in the workplace.

Types of safety concerns to report here may include:

  • General workplace and office safety
  • Potential fire hazards
  • Improper storage of hazardous materials, such as, flammables, toxic or corrosive substances
  • Anything you feel has the potential to cause harm to people, property or the environment

You may submit your concerns or findings using the
anonymous web form or by downloading and printing the PDF and mailing it directly to Health & Safety at FSC. If you would like to receive a response, please indicate your email address in either form.
